Case Management
Lifelong’s case managers are at the center of care for people living with HIV/AIDS. Case managers main role is to connect people to medical care, assist them with treatment adherence and to provide support services in order to achieve optimal health.
Chicken Soup Brigade - Lifelong's Food Program
Each week, we feed hundreds with fresh cooked meals, hearty groceries, and, of course, delicious soup! We take the traditional food bank one step further, by offering individualized nutritional therapy to people living with HIV/AIDS and other life-challenging illnesses.
Insurance
Medical care is key to living with HIV/AIDS. Lifelong’s insurance program makes it possible for those without insurance to continue seeing their doctor and receiving the medications they need to live.
Housing
It’s difficult to maintain your health when you don’t have a roof over your head. Lifelong’s housing program connects people with emergency, temporary, and permanent housing.
Access to Care (ACAP)
ACAP specializes in connecting HIV+ people with doctors and dentists that specialize in caring for people living with HIV/AIDS.
Mental Health Access Program (MHAP)
MHAP breaks down the financial barrier to clients accessing mental health care. We pay insurance co-pays for sessions with therapists who are experienced in caring for HIV positive individuals.
Treatment Adherence (STEP)
Adhering to complicated drug regimens, which often include dozens of pills each day, can be difficult, but is vital for maintaining health. Lifelong’s STEP program helps people stick to their medications, on time, every time.
